Provider Terms of Use
Version 1.0 · Updated 2026-10-20
Review draft. This document is being prepared for launch and is not yet published for acceptance.
Professional use
These terms govern individual providers using Cozy Health Limited’s provider dashboard. Provide accurate identity, practice and professional-licence information, maintain authorization to practise where applicable, and use only your own account. A dashboard verification indicator is not a clinical endorsement.
Authorized patient relationships
Access only patients with whom you have an authorized active connection, for the disclosed care-support purpose. Patient links do not enable every category: respect separate permissions, selected assessment-result grants and revocation. Do not bypass restrictions or access another provider’s patients.
Clinical responsibility and service boundaries
The dashboard supports selected wellbeing records, provider messages, notes, assigned resources and authorized reports. It is not an emergency service, diagnostic engine or replacement for clinical judgment. You remain responsible for professional duties and explaining your availability and response arrangements to patients.
Data you may access
Patient identity/link status and assignment progress may remain visible while connected. Optional sharing covers mood entries, journal summaries, assessment results and provider notes as authorized. Journal bodies, recordings and transcripts are excluded from journal-sharing endpoints. Titles and summaries can still be sensitive.
Security and downloaded copies
Protect credentials, devices and exported files. Do not share accounts, scrape data, repurpose it for advertising or redistribute it without authorization. Apply minimum necessary access and report suspected compromise to Team@cozyhealth.org.
Revocation stops subsequent authorized access; it cannot recall data you already viewed, copied or downloaded. You are responsible for lawful handling of those copies, retention obligations and secure disposal. Do not rely on this portal as your only legally required clinical record.
Patient rights and practice responsibilities
Maintain your practice’s own privacy notices and permissions. Coordinate rights requests with Team@cozyhealth.org. Where patients are minors, verify the authority and permissions applicable to the service you provide. Platform policy acceptance does not replace professional consent requirements.
Agreements, fees and termination
Execute any separately required processing or regulated-data agreement before the applicable processing begins. These terms alone do not establish HIPAA, GDPR or NDPA compliance. Consumer subscriptions do not establish provider billing terms; any provider fees require separate disclosed terms.
We may restrict unsafe or unauthorized access and investigate reports. On termination, stop using the portal and handle retained copies under applicable agreements and obligations. Submit complaints or appeals to Team@cozyhealth.org.
Acceptance and changes
Acceptance is recorded against the displayed document version and individual provider account. Material required updates require fresh review. Outstanding operator address, governing-law, retention and commercial details must be resolved before publication. Mandatory legal rights remain unaffected.
